Trulicity is a once-weekly GLP-1 injection containing dulaglutide. It is approved to improve blood sugar in adults and children 10 and older with type 2 diabetes, and to reduce major cardiovascular events in adults with type 2 diabetes who have heart disease or multiple risk factors. Its labeled uses do not include weight loss, and in head-to-head research, semaglutide lowered A1C and weight more.
What Trulicity Is
Dulaglutide is a GLP-1 receptor agonist, meaning it mimics the natural hormone that helps your body release insulin after meals. If you want the background, our guide to how GLP-1 regulates blood sugar covers the basics. It comes in single-dose pens that you inject once a week.
Approved Uses
According to the Trulicity label on DailyMed, it is indicated, along with diet and exercise, to improve glycemic control in adults and pediatric patients 10 and older with type 2 diabetes. It is also indicated to reduce the risk of cardiovascular death, nonfatal heart attack, and nonfatal stroke in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors.
Trulicity Dosing and Administration
The label describes a gradual climb. Adults start at 0.75 mg weekly and may increase to 1.5 mg after 4 weeks. Further increases come in 1.5 mg steps after at least 4 weeks on each dose, up to a maximum of 4.5 mg weekly. Children 10 and older can go up to 1.5 mg at most.
The pens come in 0.75, 1.5, 3, and 4.5 mg strengths. You can inject at any time of day, with or without food, into the abdomen, thigh, or upper arm, rotating sites each time. If you miss a dose and at least 3 days (72 hours) remain before the next one, take it as soon as possible. If less than 3 days remain, skip it and resume your regular day.
How Trulicity Compares With Ozempic
The SUSTAIN 7 trial pitted semaglutide against dulaglutide for 40 weeks in adults with type 2 diabetes. Semaglutide 0.5 mg lowered A1C by 1.5 percentage points versus 1.1 for dulaglutide 0.75 mg, and semaglutide 1 mg lowered it by 1.8 versus 1.4 for dulaglutide 1.5 mg. Weight loss was also greater with semaglutide: 4.6 kg versus 2.3 kg at the lower doses, and 6.5 kg versus 3 kg at the higher doses.
Keep in mind that this trial tested only the 0.75 mg and 1.5 mg doses, while the label now also includes 3 mg and 4.5 mg, so real-world comparisons may differ. Our article on GLP-1 and type 2 diabetes offers helpful broader context.
How Trulicity Compares With Mounjaro
Dulaglutide also served as the comparison drug in SURPASS-CVOT, a trial of 13,299 adults with type 2 diabetes and established atherosclerotic cardiovascular disease. Tirzepatide (Mounjaro) met the goal of being non-inferior to dulaglutide 1.5 mg for major cardiovascular events, with a hazard ratio of 0.92, but superiority was not established. That result helped support Mounjaro’s heart-risk approval.
Side Effects and Safety
The label’s pooled trial data show how digestive effects scale with dose. At 1.5 mg versus placebo, nausea affected 21.1% versus 5.3%, vomiting 12.7% versus 2.3%, diarrhea 12.6% versus 6.7%, abdominal pain 9.4% versus 4.9%, decreased appetite 8.6% versus 1.6%, and fatigue 5.6% versus 2.6%.
Trulicity carries a boxed warning for thyroid C-cell tumors and is contraindicated for people with a personal or family history of medullary thyroid carcinoma or MEN 2. Other warnings include pancreatitis, low blood sugar when combined with insulin or similar drugs, kidney injury from dehydration, severe digestive reactions, gallbladder disease, and allergic reactions.
