Zepbound is the brand name of tirzepatide approved by the FDA for adults with obesity, or overweight with at least one weight-related condition, to reduce excess body weight and keep it off. It is also approved to treat moderate to severe obstructive sleep apnea in adults with obesity. It is a once-weekly injection used alongside a reduced-calorie diet and more physical activity.
What Zepbound Is and How It Works
Zepbound contains tirzepatide, a dual agonist that mimics the hormones GLP-1 and GIP. Cleveland Clinic explains that both hormones are triggered by eating and increase insulin production, and tirzepatide also slows stomach emptying and reduces appetite. Mounjaro contains the same ingredient but is approved for type 2 diabetes.
Who Zepbound Is For
According to the Zepbound FDA label, it is indicated for:
- Adults with obesity, or overweight plus at least one weight-related condition, to reduce excess body weight and maintain the loss long term
- Adults with obesity who have moderate to severe obstructive sleep apnea
The label states that safety and effectiveness in pediatric patients have not been established, and that Zepbound is not recommended with other tirzepatide products or any other GLP-1 receptor agonist.
Zepbound Dosing and Forms
The label describes a start at 2.5 mg once weekly for four weeks, which is meant only for starting, followed by 5 mg. Increases then come in 2.5 mg steps, each after at least four more weeks. Maintenance doses are 5, 10, or 15 mg weekly for weight management, and 10 or 15 mg for sleep apnea, with 15 mg as the maximum.
It comes as a single-dose prefilled pen, a single-dose vial, a multi-dose vial, and a multi-dose KwikPen. If you miss a dose, take it as soon as possible within 4 days, and skip it if more than 4 days have passed. You can change your dosing day as long as at least 72 hours separate the two doses.
What Results Can You Expect?
In the SURMOUNT-1 trial, 2,539 adults with overweight or obesity and without diabetes took tirzepatide or placebo for 72 weeks alongside lifestyle support. Average weight loss was 15.0% on 5 mg, 19.5% on 10 mg, and 20.9% on 15 mg, compared with 3.1% on placebo. Results vary from person to person, and these figures are trial averages rather than guarantees.
Weight regain after stopping is a real consideration, which we cover in our guide to the role of GLP-1 in long-term weight maintenance.
Side Effects and Safety
The label’s trial data show that nausea affected 25% to 29% of adults on Zepbound versus 8% on placebo, diarrhea 19% to 23% versus 8%, and vomiting 8% to 13% versus 2%. Constipation, abdominal pain, injection site reactions, and fatigue also occurred more often than with placebo.
Zepbound has a boxed warning for thyroid C-cell tumors and is contraindicated for people with a personal or family history of medullary thyroid carcinoma or MEN 2. Other listed warnings include severe digestive reactions, kidney injury from dehydration, gallbladder disease, pancreatitis, and serious allergic reactions.
An Important Note About Birth Control Pills
The label warns that Zepbound may reduce the effectiveness of oral hormonal contraceptives because it slows stomach emptying. It advises switching to a non-oral method or adding a barrier method for 4 weeks after starting and for 4 weeks after each dose increase. Hormonal contraceptives that are not taken orally are not expected to be affected. Discuss this with your provider.
Is Zepbound the Right Choice for You?
That depends on your health history, your goals, and your coverage, and only your provider can decide.
